Here is a simple Gap Analysis Template you can use during a gap assessment with alternatives and alternative impact analysis.

This is a simple template used on a project I worked on.  It includes some of the things you may want to consider during gap analysis.  Here it is:

BUSINESS PROCESS-<Enter the Tasks to Perform a Specific Action>

GAP DESCRIPTION-<Describe the Gap>

RELATED REQUIREMENTS-<Enter Any Requirements Here>

CLIENT IMPACTED-<Name of Client>

ALTERNATIVE #1

Gap Resolution Type:<Work Around, Customization, Policy Change, Process Change>

Requires Cross-Module Integration:<Yes or No>

How Will it Work:<High Level Functional Solution>

Business Impact:<High, Medium, Low and How Much Impact to Client>

Recommendation:  <1, 2, or 3>

Gap Resolution Type:<Work Around, Customization>
